Tell us what you need and when

We answer requests in one working day. This form is not a live booking system and never will be — the length of your appointment depends on what you need, and that is a decision for a person, not a calendar widget.

About you
We ring rather than email — it's quicker.
Tells us whether you're NHS eligible.
What you need

If this is urgent, do not use this form. Flashes, a new shower of floaters, a curtain across the vision, sudden loss of vision or a red painful eye all need a phone call today — 01434 604 118.

We use what you send here to arrange your appointment and for nothing else. Clinical records are kept for ten years as the College of Optometrists requires; you can ask to see yours at any time.

Complaints, records and regulation

How to complain

Ask for Ruth Ellard, in person or on 01434 604 118. If you would rather write, reception@ellardandroe.co.uk or the address at the foot of this page. We acknowledge in three working days and answer fully within twenty, and we will tell you what we have changed as a result.

If you are not satisfied with our answer you can take it to the Optical Consumer Complaints Service, and for anything concerning a clinician's fitness to practise, to the General Optical Council directly. You do not need our permission to do either.

Your clinical records

Kept for ten years from your last visit, as the College of Optometrists requires, and for children until their twenty-fifth birthday. You can ask to see them or have a copy at any time, and we do not charge for it. Records move with you if you change practice — ask us and we will send them.

Registration and indemnity

Every clinician here is registered with the General Optical Council and their number is printed on the practice page so you can check it. Ellard & Roe is registered with the GOC as an optical business and carries professional indemnity through the Association of Optometrists.

NHS sight tests and vouchers

We hold a General Ophthalmic Services contract, so NHS sight tests are available here for anyone eligible. Optical vouchers are deducted at the current national value for your band and we show you the arithmetic on the bill rather than netting it off silently.
